then please print each form below, fill them out, and bring the completed forms with you prior to your first session, thank you.
Please note that you will need the Adobe Reader in order to view and print the PDF forms below. If Adobe Reader
is not already installed on your computer then you will need to visit Adobe's website to obtain a free copy of
Adobe Reader.
On the other hand, if you have Microsoft Word (MS Word) 2007 or newer installed on your computer
you can save the Word documents on your hard drive, fill them out using using MS Word, print the completed forms, and
bring all three (3) forms with you on your first visit to the office.
